Eli Lilly Initiated at Watchlist: The Leader in Obesity Drugs, at a Full Price

Eli Lilly and CompanyLLYReports

Eli Lilly enters coverage as the most dominant franchise in large-cap healthcare and one of the least forgiving valuations. The verdict is Watchlist. At $1,184.78, the probability-weighted fair value of $1,213 leaves about 2% of upside, and the name becomes interesting near $1,050.

The bull case

The numbers are extraordinary. Second-quarter revenue rose 48% to $23.0 billion, beating consensus by 11%, and adjusted EPS of $8.38 beat by 27%. Mounjaro and Zepbound, both built on tirzepatide, sold $14.9 billion between them, and Lilly writes about six of every ten US obesity prescriptions. Full-year revenue guidance rose to $85-87 billion, and the implied second half still looks conservative against the current run-rate.

The pipeline extends the story. Retatrutide delivered up to 20.8% weight loss at 80 weeks in patients with type 2 diabetes, and Lilly plans to file in the first quarter of 2027. The FDA also approved Onswik, the first once-weekly basal insulin in the US, on 24 September. In the bull scenario, US price cuts slow, the Foundayo pill reaches about $6 billion in 2027 sales and the stock earns 30 times earnings, which points to $1,545.

The bear case

Growth now comes from volume, while price moves the other way. Medicare’s GLP-1 Bridge began on 1 July at $50 a month to patients, self-pay pricing runs $299-449, and the CFO put the underlying US price decline at 9% in the quarter. Tirzepatide is 65% of revenue, so each further five points of US price erosion costs roughly $1.50 of annual EPS.

The oral launch is also behind. Foundayo sold $98 million in its first full quarter, short of expectations, while Novo Nordisk’s Wegovy pill sold about $494 million. Lilly has reportedly spent more than $20 billion on acquisitions this year, more than its trailing free cash flow, and its CEO has signalled larger deals ahead. Add a ten-year Treasury yield near its highest since 2007, and the bear scenario of 21 times $41 of 2027 EPS gives $861.

Where that leaves the stock

The base case of $47 in 2027 EPS at 26 times gives $1,222, close to today’s price. That makes the setup roughly symmetric for a stock priced for the good outcome. The first test comes with third-quarter results, expected in late October or early November, which should show whether US price erosion is narrowing and whether Foundayo volume is accelerating.
